Dear Dr. G,

I had a chemical peel and my skin didn’t peel.  Did it work?

Peels are some of the most important products for your skin.  Their benefits extend much deeper than the cosmetic.  Peels are an integral part of your long term treatment plan.  They are designed to fit your lifestyle and your budget.   

It is not necessary to shed skin after a peel.  Why not?  There are really two answers.

I am fully vaccinated and I am a psoriasis patient. Do you recommend the booster when I am able to receive it? My PCP tells me I can get it as soon as September 28th.

Every person in the USA has been offered readily available, free vaccination with their choice of three covid19 vaccines. I continue to recommend vaccination of those who feel it is safe to do so and who otherwise can not mitigate risk of covid19 to themselves and to others. I also support prevention of covid19 in the first place .... with simple though woefully underutilized regimens of diet, exercise, vitamins, antioxidants and asa when proper. There are a host of therapeutics that are available for those who develop covid19. These too are being terribly underused.

Dear Dr. G.,
I recently had filler and am now eligible for the vaccine. I have read that there could be side effects from the vaccine if you have filler. What are your thoughts on this?

This is a timely inquiry.  There are many types of dermal fillers …. these are injected into the skin in order to reduce fine lines and wrinkles, restore symmetry to the face, and reverse some of the signs of aging.  The results are highly dependent on who is doing the injection with knowledge, training, and experience critically important to a great result.  I use a product called calcium hydroxyapatite because it is the least reactive, and in my hands the most reliable filler available in the USA. 

The vast majority of fillers used in the USA are not calcium hydroxyapatite …. they are hyaluronic acid based.  To my knowledge only the hyaluronic acid based fillers have shown inflammation post Moderna vaccination.  They have no effect on the vaccination’s effectiveness or on the severity of “systemic reactions” to the vaccine.

The story of vitamin D is very complicated, in part because vitamin D is involved in immune function, in cancer prevention, and in maintenance of cardiovascular disease ..... in addition to its central role in calcium and bone health. Yes we each need vitamin D and with covid19 upon us, we need vitamin D more than ever. Vitamin helps us reconstitute the ACE2 receptor .... and having a healthy level of Vitamin D protects us, at least in part, from needing hospitalization from covid19.

How best to obtain vitamin D .... from a varied and healthful diet AND from vitamin D supplements.

Sun exposure (ultraviolet exposure) contributes in a major way to most skin cancers, including basal cell cancers, squamous cell cancers, malignant melanoma, and Merkel cell carcinoma. There is no "safe" minimal dose of ultraviolet light. So, when supplements are so safe and so readily available, why risk cancer while you are trying to keep your body healthy?

As I'm getting older I'm noticing more and more brown spots appearing on my face and hands...One of my good friends told me they were age or liver spots, is this true? What can I do about them?

Ageing Not like fine wine

There are lots of types of pigmented spots that can appear on the skin. Flat, sharply defined brown skin lesions that appear on mostly sun exposed skin and that are about the size of a pencil eraser are most often lentigines……a fancy word for super freckles. They make their appearance days to decades after too much sun exposure and can occur on all types of skin. In reality, they are not “age spots” because they often occur in teen agers and young adults, although they accumulate more as your numbers of birthdays accumulate. They are not “liver spots” because they have nothing to do with the liver, or with liver disease ….except for sharing some of the same color tones. If you already have lentigines please be meticulous in wearing proper sunscreen, sun protective clothing, and sun sense. Lentigines represent sun damage that has left permanent injury to skin cells.

Dear Dr. G,
My daughter vacationed in Florida this past weekend and did not reapply her Blue Lizard sunscreen I provided her, after swimming in the ocean. Thus, her skin is painfully sunburned, mostly her legs and mid section. She has been applying Aloe and taking Tylenol. Is there anything else you recommend she do or use to help in the healing?

How old is your daughter? If over 18, then aspirin 325mg by mouth every 4 hours while awake including the other recommendations. if under 18 check with pediatrician. Otherwise NO FURTHER SUN EXPOSURE and begin half strength vinegar open wet dressings three times a day followed by over the counter 1% Hydrocortisone cream three times a day. Not to mention at least double layer clothing and/or specific UV protective clothing on burnt areas.
